These Terms and Conditions govern your use of the Ketamine Wellness NY website and your engagement with our clinical services at our Queens (Jackson Heights) and Long Island (Great Neck) locations. By booking a consultation, scheduling a treatment session, or using our website, you agree to these terms. If you do not agree, please do not use our services.

Please read these terms carefully. Some provisions are legally binding.

Description of Services

Ketamine Wellness NY provides physician-supervised IV ketamine infusion therapy for treatment-resistant depression, generalized anxiety and panic disorders, PTSD and trauma, obsessive-compulsive disorder (OCD), chronic pain and fibromyalgia, and adjunctive support for alcohol use disorder. All sessions are administered in-clinic under the medical supervision of Dr. Pervaiz Qureshi, MD, with continuous RN monitoring.

Services we do not offer:

  • Spravato (esketamine nasal spray). We do not administer Spravato at either clinic.
  • Home or telehealth ketamine delivery. All treatments require an in-clinic visit.
  • Emergency medical services. Ketamine Wellness NY is not an emergency service. If you are experiencing a psychiatric or medical emergency, call 911 or the 988 Suicide and Crisis Lifeline.
  • Direct insurance billing. We are cash-pay only. We do not bill your insurance carrier.
  • Insurance appeals or prior authorization services.

Off-Label Use Disclosure

IV ketamine is FDA-approved as an anesthetic. Its use for the psychiatric indications listed above (depression, anxiety, PTSD, OCD, and chronic pain) is considered off-label. Off-label use is legal and clinically common when supported by published evidence and medical judgment, and is a standard practice for IV ketamine therapy. Our clinical team will discuss the specific evidence for your condition during your initial consultation.

Spravato (esketamine nasal spray), which we do not administer, is separately FDA-approved for treatment-resistant depression at REMS-certified clinics.

Ketamine as a Controlled Substance

Ketamine is a Schedule III controlled substance under federal law. When administered at the low, medically supervised doses used in our treatment protocol, the risk of dependence is considered very low. However, patients with a history of ketamine or ketamine-analog abuse, or active substance use disorder without stability, will be screened during the initial consultation and may not be candidates for treatment.

Eligibility Requirements

You must be 18 years of age or older to receive treatment at Ketamine Wellness NY. We do not treat patients under 18.

You must complete an initial clinical consultation before your first infusion. During the consultation, our team will review your medical history, current medications, and treatment goals to determine whether IV ketamine therapy is appropriate for you.

You may not be a candidate for treatment if you have:

  • Active psychosis or schizophrenia
  • Uncontrolled hypertension without cardiology clearance
  • Certain cardiovascular conditions (evaluated case by case)
  • Active substance use disorder without documented stability
  • Pregnancy or breastfeeding without prior clearance from your prenatal provider (evaluated case by case)

Our medical team makes the final determination on eligibility during your consultation.

Initial Consultation Requirement

An initial clinical consultation is required before your first treatment session. The consultation includes a comprehensive review of your medical and mental health history, current medications, treatment goals, and eligibility screening. It is included in the price of your introductory session ($450) and is not billed separately.

If, during or after the consultation, you decide not to proceed with treatment, you may decline the infusion. In that case, our standard consultation-only rate applies, which our team will discuss with you upfront.

Consent to Treatment

By scheduling a treatment session, you acknowledge and agree that:

  • You have been informed of the off-label nature of IV ketamine for psychiatric and chronic pain indications.
  • You understand that IV ketamine produces dissociative and perceptual effects during the session, which are expected and monitored by our clinical team.
  • You have been informed of common side effects, which may include nausea, temporary blood pressure elevation, dizziness, drowsiness, headache, or brief disorientation.
  • You understand that individual clinical response varies. No specific outcome is guaranteed.
  • You have arranged transportation home from the clinic. You may NOT drive yourself home after a session. Ridesharing, public transit with a companion, or being picked up by a designated driver are all acceptable.
  • You will disclose all current medications, medical conditions, and substance use during the intake process. Failure to disclose can affect your safety during treatment.
  • You may withdraw consent and stop the infusion at any time during the session.

A separate written Consent to Treatment form will be provided at your first visit and must be signed before any infusion.

Payment Terms

 

  • Introductory Session: $450 (new patients only, includes intake and infusion)
  • Standard Single Session: $550
  • 6-Session Protocol: $3,000

Payment is due at the time of service. We accept:

  • Credit cards, debit cards, and cash
  • HSA (Health Savings Account) and FSA (Flexible Spending Account) cards
  • The 6-session package is paid in full at the start of your treatment course

We do NOT accept insurance for direct billing. We do NOT bill Medicare, Medicaid, or commercial insurers. We do NOT handle prior authorizations or insurance appeals.

Superbill Provision. At the end of each session, we provide an itemized superbill including CPT codes (99215 with modifier 25 for the office visit, 96365 for the IV infusion, and 99203 for your initial evaluation when applicable), ICD-10 diagnosis codes documented by our clinical team, the date of service, and Dr. Qureshi’s provider NPI. You may submit this superbill directly to your insurer as an out-of-network mental health claim. Reimbursement outcomes vary by plan and are not guaranteed.

Cancellation, Rescheduling, and No-Show Policy

We ask that you provide as much advance notice as possible when cancelling or rescheduling a session.

  • 24+ hours advance notice: Full refund of any prepayment. No fee.
  • Less than 24 hours advance notice: 50 percent of the session fee will be charged. Package sessions will be counted toward your total.
  • No-show: 100 percent of the session fee will be charged. Package sessions will be counted toward your total.

To cancel or reschedule, call (516) 203-1041 during business hours or leave a voicemail with your name and appointment date.

Late Arrival Policy

Please arrive on time or a few minutes early for your scheduled session. If you arrive more than 20 minutes late, we may not be able to accommodate you and the appointment may be rescheduled at our discretion. In that case, the missed session will be billed as a no-show under Section 8.

If you are running late, please call (516) 203-1041 immediately so we can adjust if possible.

Insurance Clarification

Ketamine Wellness NY is a cash-pay clinic. What this means for you:

  • We do not bill your insurance directly.
  • We do not verify your insurance benefits before treatment.
  • We do not handle prior authorization or insurance appeals.
  • We do provide superbills you can submit to your insurance for potential out-of-network reimbursement (see Section 7).
  • Reimbursement outcomes are between you and your insurer. We cannot guarantee any level of reimbursement.

If insurance-covered treatment is essential to your budget, Spravato at a REMS-certified clinic is your best path. Our team can direct you to the resources you need to find one.

Referrals

Our clinical team may recommend referrals to outside providers for services we do not offer, including:

  • Integration therapists (for talk therapy paired with ketamine treatment)
  • Spravato providers at REMS-certified clinics
  • Primary care or psychiatric providers for ongoing medication management
  • Other specialists as clinically indicated

We do not accept referral fees, kickbacks, or any other compensation from providers we refer to. Referrals are made solely based on clinical fit for your care.

No Guarantee of Outcome

We are committed to delivering evidence-based IV ketamine therapy in a safe, professional environment. However, individual clinical outcomes vary. We do not guarantee any specific therapeutic result, level of symptom improvement, duration of relief, or side effect profile.

Emergency Procedures

Ketamine Wellness NY is not an emergency medical service.

  • If you are experiencing a psychiatric emergency (including suicidal thoughts, active self-harm, or acute crisis), call the 988 Suicide and Crisis Lifeline or go to your nearest emergency room.
  • If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
  • For after-hours clinical questions related to a recent session, call (516) 203-1041 and follow the after-hours prompts.

Privacy and Protected Health Information

Your medical records and personal information are protected under the Health Insurance Portability and Accountability Act (HIPAA). Please review our full Privacy Policy for complete details on how we collect, use, share, and protect your information, and your patient rights under HIPAA.

Intellectual Property

All content on our website (including text, graphics, logos, images, and video) is owned by Ketamine Wellness NY or our licensors and is protected by copyright, trademark, and other intellectual property laws. You may not copy, reproduce, distribute, or create derivative works without our written permission.

Limitation of Liability

To the fullest extent permitted by law, Ketamine Wellness NY, its physicians, nurses, staff, owners, and affiliates are not liable for any indirect, incidental, or consequential damages arising from your use of our website or services, EXCEPT to the extent that limitation is prohibited by New York State law.

This limitation does not affect any rights you have under HIPAA, New York State health law, or applicable federal or state consumer protection statutes. It does not apply to gross negligence, willful misconduct, or intentional harm.

Governing Law and Jurisdiction

These Terms and Conditions are governed by the laws of the State of New York, without regard to conflict-of-law principles. Any disputes will be resolved in the state or federal courts located in Queens County, New York.

Modification of Terms

We may update these Terms and Conditions from time to time. When we make material changes, we will post the updated terms on this page with a new “Last Updated” date at the top. Your continued use of our services after any change constitutes acceptance of the updated terms.

For material changes affecting patient safety, informed consent, or pricing, we will provide additional notice.

Severability

If any provision of these Terms and Conditions is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will continue in full force.
